Position Description

A rehabilitation services organization on Long Island is currently seeking Licensed Practical Nurses (LPN) to join one of their skilled nursing facilities in Suffolk County.

About the Opportunity:

Setting: Skilled Nursing Facility
Shifts: Days, Evenings, Nights
Schedule: 4-5 shifts per week
Hours: 7am to 3pm or 3pm or 11pm or 11p-7a (Open to double 16 hour shifts)
Location: Yaphank, New York

Responsibilities:

Assist residents with activities of daily living (ADLs): including bathing, dressing, grooming, toileting, and feeding.
Monitor and report changes in residents' conditions: such as vital signs, behavior, or mobility changes to nurses or supervisors.
Provide mobility support
Maintain a clean and safe environment: including changing bed linens, cleaning resident areas, and ensuring equipment is sanitized.
Document care provided: accurately record daily tasks and any observed changes in the resident's physical or mental condition.

Qualifications:

Graduate of an accredited School of Nursing
NYS Licensed Practical Nurse (LPN)
Solid assessment and documentation skills
Great interpersonal skills
Excellent communication skills (written and verbal)
Strong attention to detail
Highly organized
